Choose the care approach that works best for you—whether through regular text updates or monthly phone calls, our team is here to support your pain management journey.

Connect – Remote Therapeutic Monitoring (RTM)

Remote therapeutic monitoring engages you through regular text message questions, the information you provide enables greater insight into your care plan and allows us to adapt your care in real-time. Text messages are sent regularly asking about your care.

Questions include:

  • Pain score assessments
  • Feedback regarding effectiveness of medications and other therapeutic interventions like procedures and physical therapy
  • Assessment of self-management strategies

When you reply to these messages, we can track how well you are responding to your care plan and if any adjustments are needed to provide the highest quality care possible.

Connect – Principal Care Management (PCM)

Connect Principal Care Management (PCM) uses comprehensive care management assessments by clinical staff who engage with you over the phone. Our friendly nurses review trends in your pain and discuss the effectiveness of your treatment plan in a convenient monthly phone call.

This program does not utilize text messaging as the primary mode of communication. You will have the option to text our nurses if you have a care need, but the program requires you to initiate real-time care needs.

Regardless of the program, if you do choose to text, the messages will be responded to within 24 hours (if M-F) and all messages sent over the weekend will be responded to the next business day in which the practice is open. You can call our nursing line 24/7. If that phone is not answered in real-time, please leave a voicemail and someone will call you back as quickly as possible.
